The purpose of this form is to support clinical safety, professional growth, and healthy supervisory relationships—while ensuring counsellors and clients are protected. It can be used when supervision feels unhelpful, unsafe, or difficult to raise in person, including concerns related to ethics, boundaries, communication, or power dynamics. All reports are reviewed thoughtfully and in line with our professional and ethical standards.
This form provides a safe and confidential way to raise concerns about professional or ethical practice in counselling by counsellors or clients. Submissions are confidential, and you may choose to remain anonymous. Our intention is to support ethical practice, safeguard clients and counsellors, and strengthen professional standards.
